Notification of Data Security Incident

Floresville, TX - In an abundance of caution, Connally Memorial Medical Center (CMMC) is providing notice of a recent privacy incident. CMMC recently discovered unauthorized access to a single employee email account and immediately worked to ensure that the affected account had been properly secured and that the incident was fully remediated. In addition, CMMC swiftly engaged a third-party team of forensic investigators in order to determine the full nature and scope of the incident. Following a thorough investigation, on July 29, 2024, CMMC confirmed that a limited amount of information may have been at risk in connection with this incident.
Although the forensic investigation could not rule out the possibility that an unknown actor may have accessed this information, there is no indication whatsoever that any information has been misused. The types of information contained within the affected data included name, address, date of birth, Social Security number, driver's license number, medical record number, patient ID or account number, Medicare or Medicaid number, health insurance information, medical diagnosis and treatment information, doctor or facility name and/or date of service. Importantly, the information potentially affected may vary for each individual, and may include all, or just one, of the above-listed types of information.
In response to this incident, CMMC has taken steps to further enhance the security measures within its network and is reviewing its current policies and procedures related to data security. Additionally, CMMC is offering potentially affected individuals complimentary credit monitoring and identity protection services through CyberScout, a TransUnion company. Should individuals have questions about the incident, they may contact a dedicated assistance line at 1-844-995-6275 between 8:00 a.m. to 8:00 p.m. ET, Monday through Friday, excluding holidays. Individuals may also contact CMMC by writing to 499 10(th) St., Floresville, TX 78114. Although CMMC has no evidence of actual misuse of information as a result of this incident, individuals are nonetheless encouraged to monitor their account statements and explanation of benefits forms for suspicious activity and to detect errors. To order a free credit report, individuals may visit www.annualcreditreport.com or call 1-877-322-8228. Individuals may also place a fraud alert or credit freeze by contacting the credit reporting agencies: TransUnion at 1-800-680-7289; Experian at 1-888-397-3742; Equifax at 1-888-298-0045. Individuals can further educate themselves regarding identity theft, fraud alerts, credit freezes, and steps to protect their personal information by contacting the credit reporting bureaus, the FTC, or their state Attorney General.
