Notice of Privacy Incident March 14, 2025

Nashville, Tennessee - CDHA Management, LLC and Spark DSO, LLC dba Chord Specialty Dental Partners ("Chord") are providing notification of a recent data security incident that may have impacted certain information related to its patients.
On or around September 11, 2024, CDHA Management, LLC and Spark DSO, LLC dba Chord Specialty Dental Partners ("Chord") discovered suspicious activity related to an employee's email account. Upon discovery, Chord took immediate action to secure the account and engaged a team of third-party specialists to assist with determining the full nature and scope of the incident. The investigation determined that an unauthorized individual had gained access to several accounts for a limited time between August 19, 2024, to September 25, 2024. Upon confirmation of unauthorized access to the email accounts, Chord commenced an investigation to determine whether the affected accounts contained sensitive information pertaining to individuals. On January 15, 2025, Chord confirmed that a limited amount of personal information may have been accessed by an unauthorized party in connection with this incident.
The types of information contained within the affected accounts may have included address, Social Security number, driver's license, bank account information, payment card information, date of birth, medical information, and health insurance information. Importantly, the potentially impacted information may vary for each individual and may include all or just one of the above-listed types of information.
At this time, there is no evidence indicating any information was subject to actual or attempted fraudulent misuse as a result of this incident. In an abundance of caution, Chord conducted a comprehensive review of the potentially affected information to identify and subsequently notify all potentially affected individuals.
Chord will be providing written notice via US Mail to potentially impacted individuals with additional details. As an additional precaution, Chord will offer potentially impacted individuals access to credit monitoring and identity protection services. If individuals have questions about this incident, they may contact a dedicated assistance line at 1-833-998-6327, Monday through Friday from 8:00 AM to 8:00 PM EST (excluding major U.S. holidays). Individuals may also place a fraud alert or credit freeze by contacting the credit reporting agencies: TransUnion 1-800-680-7289; Experian 1-888-397-3742; Equifax 1-888-298-0045.
